The survey assumes that only one major location is involved. If you would like to provide information for additional locations or facilities, we would welcome your input of their data by using separate questionnaires. We have designed this questionnaire to be as easy to complete as possible. Each section and topic in the survey is important and provides valuable information to employers when the survey results are published. Some questions allow for responses in the following three categories:
Hourly=Hourly Employees
Office=Office - Technical (Salaried Non-Exempt)
Management=Management (Salaried Exempt)
